Kiírta és kiadta 1834. október 27-én Márton József prédikátor. Összeolvasván az Anyakönyvvel hitelesítette Budaházy Ferentz, a megye szolgabirája és eskütje Simonffy Ferentz. Az aláírások mellett viaszpecsétek ill. az iraton ép illetékbélyeg is található. 380 X 248 mm.
Vízjeles, merített papíron, hajtogatva. Néhány helyen a hajtogatás elrepedve.
