Hőgyes Endre aláírásával. M: 62 x 46 cm. A Budapesti Tudományegyetem pergamen diplomája, fatokos függőpecséttel, Lévius Károly gyógyszerész nevére kiállítva.
Verzóján a Bars megyei főorvos, illetve Léva polgármesterének hitelesítő aláírásával 1894-ből.
Lévius Károly budaörsi patikájáról említést tesz a Lengyel Júlia: Pest megye polgári gyógyszerészetének története a XVIII-XIX. században" c. mű (1999 - 126. l.)."
